2026 Rural Community Health Fund Grantees

Investing in Rural Health: Meet the 2026 Rural Community Health Fund Grantees

The Greater Fayette Community Foundation is proud to celebrate four organizations selected as 2026 Rural Community Health Fund grantees. Together, these organizations received $90,000 to support projects addressing health, stability, connection, and other social drivers of health across rural South Central Texas.

This year’s grantees are investing in approaches that meet people where they are. From outdoor wellness and peer support to housing stability, youth opportunity, and stronger community connections, each project reflects the knowledge and leadership already present within our rural communities.

Pfaith House | Service Area: Bastrop and Williamson Counties

Pfaith House will provide early intervention and wraparound support for women, children, and families by addressing housing stability, maternal health, behavioral health, and other social drivers of health. Through individualized care, resource navigation, and community partnerships, the initiative will help families build resilience, improve wellness, and work toward long-term stability.

Caldwell County Community Foundation | Service Area: Caldwell County

Caldwell County Community Foundation will expand the Caldwell County Opportunity Youth Collaborative to reconnect youth ages 16 to 24 who are disconnected from school and work. The initiative will connect participants with education, employment, mental health, leadership, and community resources while strengthening youth outreach, partner coordination, and youth leadership.

Breaking Bread Outreach Ministries | Service Area: Hays County

Breaking Bread Outreach Ministries will expand its Stability Support Pilot, a relational model designed to help individuals and families navigate housing instability, economic hardship, and other barriers to long-term well-being. Through resource navigation, consistent support, and community partnerships, the program will connect families to services while strengthening stability, self-sufficiency, and community connection.

Bastrop County African American Cultural Center | Service Area: Bastrop County

Bastrop County African American Cultural Center will pilot a shared Community Engagement Coordinator between the Bastrop County African American Cultural Center and Kerr Community Center. The coordinator will support resident engagement, volunteer development, partnerships, and health and wellness initiatives while helping evaluate whether a shared leadership model can strengthen community connectivity and well-being in rural Bastrop County.

Congratulations to each of our 2026 Rural Community Health Fund grantees. Their work reflects the power of locally informed solutions and the importance of investing in the people and organizations closest to community needs.

We are grateful to the community members, partners, and reviewers who help make this community-led funding possible. Together, these investments are helping strengthen health, connection, and opportunity throughout rural South Central Texas.
